FALL 2016 CONDITIONALLY ADMITTED CHECKLIST
YOUR ADMITTANCE TO THE SCHOOL OF NURSING IS CONDITIONAL UPON COMPLETION OF THE FOLLOWING DEADLINES.
Criminal Background Check - Deadline: 5:00PM on Friday, June 17, 2016
Submit to the SON Admission’s Office your blue postcard (not a copy) OR a copy of your Declaratory Order letter from the Texas Board of Nursing. Your background check is conducted by the Board of Nursing and once cleared signifies you are eligible to sit for the NCLEX-RN® after graduation barring future incidents.
Laptop Inspection - Deadline: Wednesday, August 25, 2016. Please schedule your laptop appointment between Wednesday, June 8, 2016 and Wednesday, August 24, 2016.
There are two documents listed below, Laptop Inspection Details and Student Laptop Inspection, you will need to download. You must take Student Laptop Inspection form with you to your inspection. Having a laptop that meets or exceeds the minimum specifications is a requirement to be in the nursing program.
Creating Your Certified Background account and uploading completed documents - Download Certified Background (Castle Branch) Instructions below.
At the St. David’s School of Nursing students use Certified Background to electronically manage their nursing program documents. A unique feature about Certified Background is that you will have this electronic account forever. For example, when you applying for your first and future nursing positions, you can securely upload and share documents such as your resume and portfolio, certifications and references. The one-time fee to electronically manage your nursing documents is $28; this fee is paid directly to Certified Background. You must create your account before you upload completed documents.
The deadline is 5:00PM on Monday, July 18, 2016 to upload the following completed documents onto your Certified Background account (download documents below).

- Upload a copy of your Basic Life Support certification (CPR) for Healthcare Provider completion card or ECard. See your Certified Background account for details regarding acceptable certification.
- We suggest you complete this training in May, June or early July to avoid renewing it when you are very busy preparing for graduation.
- Upload documentation of Health Insurance
- Provide a copy of your current health insurance card or proof of coverage. This insurance must cover you through the 2017 summer school semester.
- If you do not have health insurance, there are many resources on the internet.

Optional Fitting Day - Wednesday, June 22, 2016 from 10AM to 2PM
If you would like to try-on your required scrubs, polo shirt and optional shirt before you actually place your order – swing-by the Nursing Building in Round Rock between 10AM to 2PM. When you enter the building – there will be signs directing you where to go. We will have samples of all sizes and representatives will help you with a good fit. No need for an appointment. For details about the required scrubs and polo shirt – see the section of this outline called Clinical supplies, shoes, scrubs and polo shirt.
2016 ATI fee - $541 - Deadline to pay: 5:00 PM on Monday, August 1, 2016
The St. David’s SON provides you with many resources to be successful in nursing school. One resource is ATI online resources and testing. You will access these online resources to clarify complex processes you are studying. Students often benefit from watching videos or reading about these processes in a different textbook. ATI provides you with these resources throughout nursing school. Each semester you will also take a national ATI assessment which compares your academic progress to nursing students across the nation.
Your two ATI fees ($541 and $359) also includes a 3-day mandatory NCLEX-RN Review after you complete your senior year. The first week in July you will receive directions to pay the first ATI fee.
Early in the fall semester, faculty member Ms. Finney will demonstrate ATI resources using one of your course syllabi as a guide. Your second ATI fee ($359) is due in August 2017.
School of Nursing Online Day One - Deadline to complete: 1:00PM on Sunday, August 7, 2016
In mid-July 2016 you will receive instructions to complete Online Day One. Online Day One will be available to you from Saturday, July 23, 2016 at 8:00AM to 1:00PM on Sunday, August 7, 2016.
There are two parts to 2016 School of Nursing Online Day One. The first part includes reading
2016-2017 BSN Student Handbook and uploading four completed forms onto your Certified Background account.
Second Background Check and Drug Screen - Deadline to complete: Friday, August 12, 2016
As an incoming nursing student, you will complete a second Criminal Background test and drug screen through Certified Background. Per our hospital affiliation agreements, a second criminal background test and drug screen must be completed either the week of August 1 or the week of August 8, 2016. Instructions will be forwarded to you in mid-July.
Do not pay for these two items (TG75 – Background Check + Drug Test) until the first week of August.
Clinical supplies, shoes, scrubs and polo shirt
You will need these items for the first day of fall 2016 semester – Monday, August 29, 2016.
- Stethoscope with ID tag – adult size mid-price range; you may select any color tubing
- Blood pressure cuff - adult size mid-price range; you may select any color
- Kelly clamp
- Bandage scissors
- Penlight – you may select any color
If you need help figuring out which brand of the above clinical supplies to purchase (for example: Littmann stethoscope vs. another brand), consider purchasing them through MDF Instruments. The supplies listed on this website have been approved by nursing faculty:
Note: All of the letters in the above link must be in lower case.
Heads Up: A small portion of each sale goes to the St. David’s School of Nursing – Student Nurses Organization.
All white or all black, non-permeable (preferable leather) closed toe and closed heel are required. Shoes may not have mesh or any other color on them (including logos). Select shoes which give your feet and body support.
Scrubs, polo shirt and other shirts
The company that makes our scrubs is having a challenging time with the dye of the material – the color fades rapidly. Until this is corrected – our orders cannot be filled. When the dye is corrected – you will receive an email which will include an order form.
When the dye situation is corrected - order:
- scrub top: Tops run a little small – order the next size
- A scrub pants: Pants run a little small – order the next size
- Polo shirt Tip for women: The polo women’s shirts are too small and run short - order a men’s polo.
If you are cold natured or have tattoos on your arms – you will want to also order a long sleeve shirt. You may only wear these shirts under your scrubs for both simulation and clinicals.
If you have tattoos on your neck – you are to wear a white turtle neck shirt in both simulation and clinical settings. This shirt is not offered through the scrubs company.
You do not need to order a lab coat.
SON Day One - Thursday, August 25, 2016
Attendance to the SON Day One is mandatory. Generally students arrive at 8:30AM and the activities end at 5:00PM. Parents and loved ones are invited to the welcome session (9-10AM). A detailed schedule will be sent to you in late July.
Annual Scroll Ceremony - Saturday, September 17, 2016
The College of Health Professions will hold the annual Scroll Ceremony on Saturday, September 17, 2016 in San Marcos. The Ceremony emphasizes the importance of compassionate care as well as scientific proficiency. Texas State University students from all the health professions programs join together to pledge an oath to society to provide compassionate, high quality care to their future patients. Attendance is mandatory.
The St. David’s School of Nursing is committed to meeting the needs of students with disabilities. Please note the process for accessing some services and accommodations at the St. David’s School of Nursing (SON) will differ from those described for Texas State University in San Marcos.

You will be asked to provide documentation from the Office of Disability Services. Failure to contact your course faculty in a timely manner may delay your accommodation(s).
