Fall 2020 Conditionally Admitted Checklist


Please note:  Due to COVID 19, dates are subject to change.  Please continue to come back to this checklist to be sure you have the most current information.

This check list is not complete and more dates and deadlines will be added as they become available.  


UPDATE:  For SON students, fall lectures will be held online, exams in person, labs and clinical in person (1-2 times per week).  Because you will be required to be on campus during this time please plan your living arrangements in the area. 

The SON will be taking measures for your safety.  Masks will be required, surfaces sanitized and social distancing measures will be in place during fall 2020 in our facility.

  • Conditional Admission Acceptance:

    Deadline: Friday, April 10, 2020 at 5PM

    Send an email to Staci Strauch ( accepting the offer of conditional admission or alternate status to the St. David’s School of Nursing. If we do not receive your confirmation by this deadline, we will assume you do not intend to enroll.

  • Admission Deposit:

    Deadline:  Friday, April 24, 2020 at 5PM

    Payment of the admission deposit confirms your continued progress in the admittance process. 

    Submit a non-refundable, electronic payment of $145.00 HERE. Your deposit pays for exam fees and professional liability insurance while in nursing school.

    Only electronic payments are accepted.

    If we do not receive your admission deposit by the deadline, you will be removed from our conditionally admitted student list.  





  • Criminal Background Check and Fingerprints (Texas Board of Nursing):

    Deadline:  Friday, June 12, 2020 at 5PM for receipt of blue card or documents.

    Deadline for your first BON criminal background check including fingerprints. The SON Admission’s Office must be in receipt of either your blue postcard, Declaratory Order or letter from the Enforcement Department. Instructions were emailed to you earlier this year.

    Note:  The traditional B.S.N. student will undergo two background checks. The first criminal background check conducted through the Board of Nursing is required prior to admission to the St. David’s School of Nursing at Texas State. The second background check, conducted by a third-party vendor, Clinical Student, is required by our clinical partners and a valid social security number must be submitted. Students must complete this background check and drug screen as part of the admission process. Throughout the program, nursing students are subject to random or for cause drug and alcohol screens.

  • Basic Life Support Certification:

    The deadline is 5:00PM on Monday, August 17, 2020 to upload a copy of either your completion card or ECard into Clinical Student.

    It is required that you attain Basic Life Support certification (CPR plus AED training) for Healthcare Providers. You will need to renew this training every two years.   We suggest you complete this training in May, June or early July to avoid renewing it when you are very busy preparing for graduation.

    You will be required to get BLS certified and must take care of this requirement before starting classes. You can choose to attend one of the following sessions with SON to complete your certification. The course costs $65.00 per person.  The fee includes an e-book, course, and e-card.

    **  Dates for BLS at the SON have passed, future dates are being considered. Please check back. 

    The ONLY acceptable cards and valid online options for Basic Life Support training are below:

    A. American Heart Association (AHA) "Healthcare Provider (HCP)" card (two year certification).


    American Red Cross "CPR Professional Rescuer & AED for Healthcare Providers" card (two-year certification).


    The Military Training Network (MTN) BLS for Healthcare Providers card--an AHA affiliate (two-year certification).

    All other cards are invalid, and holders must complete an Approved American Heart Association course.  If AED training is not included in the title of the course, submit course outline or description.   Cards not accepted include: American Safety and Health Institute (AHSI), CPR Resources, Texas CPR Training, American CPR Training and any other cards other than the above listed cards. The statement Taught according to AHA guidelines or some variation of this statement does not make the card acceptable.

    **If your approved BLS certification expires while you are in nursing school, upload a copy of either your completion card or Ecard. The renewal of this requirement must meet the requirements identified for initial BLS for Healthcare Providers Certification

    If you hold current BLS certification, please email Dr. Chris McClanahan ( to confirm it can be accepted and if you will need to retake the certification or not before starting the program this fall. 



  • Information about uploading your nursing program documents to Clinical Student will be added here soon. Thank you for your patience as we work to get this information to you. For now, please see below for some updated information.


    You will be getting an invitation soon to enroll in Clinical Student.  This site will be used to track all clinical screenings, background checks, and required forms. Please do not ignore this email (and be sure to check your junk mail) as deadlines are associated with this and can affect your conditional status. Also, you will have access to everything one year after you graduate and up to five years total from the time you start your program. This will allow you to use your information for interviews after graduation.

    The cost to sign up for Clinical Student is approximately $125. Two of the forms you will be required to complete and upload to Clinical Student are the 2020 Health Certificate form and the 2020 Immunization & Tests form which will require a screening from a doctor. These forms will be sent to you via email. Please begin working on completing this requirement. Once complete, please save these in a safe place so that they can be uploaded once you have access to Clinical Student.

    In addition, you may also begin completing your Student Emergency Contact form (also emailed to you). More forms will be required and explained once you enter Clinical Student, but for now, you can start on these.

    The deadline is 5:00 PM on Monday, August 17, 2020 to upload your completed 2020 Immunizations and Tests Form. Upload the completed form to this requirement.
    The only document uploaded here is your completed 2-page SON 2020 Immunizations and Tests Form. Do not upload copies of your immunization records

    The deadline is 5:00 PM on Monday, August 17, 2020 to upload your completed 2020 Health Certificate. 

    • Make an appointment with your healthcare provider.  If your provider does not have your documentation of the required immunizations/tests, bring your records with you.  Also bring the 2020 Immunizations and Tests Form and the 2020 Health Certificate to this appointment for your healthcare provider to complete. 

    The deadline is 5:00 PM on Monday, August 17, 2020 to upload your completed 


    Note:  Do not upload copies of your immunization records


    The required Hep B series takes 6 months to administer, so please work on this ASAP!


    If you did not receive your third Hep B vaccination by the 5:00 PM on Monday, August 17, 2020 deadline, upload documentation here when the third vaccination is completed.  Documentation must include:

    • Your name
    • Date of birth
    • Name of vaccine
    • Date vaccine was administered
    • Dose
    • Injection site
    • Lot #
    • Manufacturer of vaccine
    • Date of expiration
    • Signature of vaccine administrator


    Your Tetanus vaccine is good for 10 years.  If your Tetanus (TDAP) vaccine expires while you are attending nursing school – upload your documentation here.  Documentation must include:

    • Your name
    • Date of birth
    • Name of vaccine:  TDAP (Td is not acceptable)
    • Date vaccine was administered
    • Dose
    • Injection site
    • Lot #
    • Manufacturer of vaccine
    • Date of expiration
    • Signature of vaccine administrator

    ANNUAL INFLUENZA VACCINATION   (do not do this before Sept. 1)

    You are required to receive your annual flu vaccine between during the months of September and October. The deadline to upload documentation of your 2020 flu vaccine will be provided once fall semester begins.  If you get your vaccine before September 1, 2020 – you may need to repeat this vaccine during the above stated dates. Documentation must include:

    • Your name
    • Date of birth
    • Name of vaccine
    • Date vaccine was administered
    • Dose
    • Injection site
    • Lot #
    • Manufacturer of vaccine
    • Date of expiration
    • Signature of vaccine administrator



    Annual TB screening is required by all students. You should note the due date of your TB renewal on your calendar and complete the screening prior to the expiration date.

    Documentation must include:

    TB Blood Test:   T-Spot or QuantiFERon

    Documentation must include:

    *  Your name

    *  Date of birth

    *  Name of blood test

    *  Date of test (dd/mm/year)

    *  Result of blood test         

    *  Signature of vaccine administrator


    If you previously tested positive for TB, complete the Tuberculosis System Screening Form; contact the SON for this form. Documentation needs to include your name, date of birth, updated details of your treatment plan, and your healthcare providers printed name, signature and their business or agency.



  • School of Nursing (SON) Online Orientation

    Start:  Saturday, July 18, 2020 at 12AM

    Deadline:  Sunday,  August 2, 2020 at 11:59 PM

    Deadline to Upload ALL Forms to Castle Branch:  05:00 PM, Monday, August 17, 2020 

    In mid-July 2020 you will receive instructions to complete Online Orientation. This required Online Orientation will be available to you from Saturday, July 18, 2020 at 12:00AM and closes on Sunday, August 2, 2020 at 11:59PM.

    All questions/quizzes must be answered by this deadline. Failure to do so will result in elimination from the program.

    There are two parts to the  2020 School of Nursing Online Orientation. The first part includes reading the 2020-2021 BSN Student Handbook, correctly answering a series of questions and uploading four completed documents into your Castle Branch account. All documents to be completed are found in your Castle Branch account.  The second part is a retention survey and does not include documents which need to be completed and uploaded onto your account.


    Document A: Student Handbook Verification Statement
    The deadline is 5:00 PM on Monday, August 17, 2020 to upload your completed Statement into your Castle Branch account

    Document B: Confidentiality Agreement - Texas State University - SON
    The deadline is 5:00 PM on Monday, August 17, 2020 to upload your completed Agreement into your Castle Branch account. 

    Document C: Conditions That May Disqualify Nursing School Graduates From Licensure and of Their Rights to Petition the Texas Board of Nursing for a Declaratory Order of Eligibility Document
    The deadline is 5:00 PM on Monday, August 17, 2020 to upload your completed Conditions into your Castle Branch account; upload both pages.

    Document D: Photo/Video Release Form
    The deadline is 5:00 PM on Monday, August 17, 2020 to upload your completed Form into your Castle Branch account. 

    You will also be asked to upload a  photo during online orientation to be used on your clinical badge.   Instructions for the photo will be available in the  online orientation. 







  • Start: June 22, 2020

    Deadline: Wednesday,  August 19,2020, at 5pm. (deadline for completed inspection document to be uploaded to Castle Branch)

    Your laptop is required to be inspected to ensure that you are equipped with all proper functions for class lectures, exams, labs and clinicals. This inspection will be in collaboration with ITAC. The deadline to complete (and pass) your inspection and to upload your completed Inspection document into Clinical Student is 5PM on Wednesday, August 19, 2020.

    All incoming nursing students are required to have a laptop for the duration of the nursing program; passing a laptop inspection is a condition of admission.Once you have completed this inspection, you will be required to upload your inspection verification to Castle Branch. Please start conducting these appointments now and do not wait until the first day of school as ITAC will get overwhelmed and you will be required to wait. Their preference is also that you use their helpdesk or live chat. An inspection can be performed remotely and the form would be emailed to you once completed.  To start the process, please call 512-245-4822 or visit You can also email and a ticket will be generated. Someone will then reach out to you to setup an appointment for the inspection.

    Given the COVID-19 situation, this is probably the best way we can recommend having an inspection performed to aide in promoting social distancing as well.  The laptop inspection form has also been emailed to you so that you can arrange to get this done during this time. The form outlines all the hardware requirements you will need to abide by in order to have a successful experience during your time in the Nursing program.  When your laptop successfully passes inspection, upload your completed Student Computer Inspection  2020 document into your Castle Branch account.    Please do not hesitate to get this done!



  • 2020-2021 Student Emergency Contact Information.

    Health Insurance Documentation (annual requirement) 

    The deadline is 5pm on Wednesday  August 19, 2020 to upload a copy of your current health insurance card or proof of coverage into Clinical Student. This insurance must cover you for the:
    2020 fall semester
    2021 spring semester
    2021 summer school semesters
    If your insurance card does not include your name – ask a representative to provide you with a letter which includes your name. Upload this documentation to this requirement.

    Option 1: University Bookstore (Preferred and Recommended Option)

    1. This is where the bundle is…(Book Bundle from TXST Bookstore) not the Elsevier website. If you purchase this option, it will cost $694.50 and you will select TX State Sem 1 PE Fall 2020 (custom). In addition, you will also need to purchase the Essentials of Nursing Practice Package (3 book set) for $99.00. This brings your total to roughly $793.50 before taxes.
    2. This bundle includes all the books that were listed as required and more. They are also listed in the handout attached named Book Bundle from TXST Bookstore. This bundle also includes several supplemental e-resources and access to e-guide books and study guides also featured on the handout that will be helpful to you as you study and prepare for exams.
    3. Currently this bundle is on backorder at our bookstore, but they should be arriving next week. This is only an estimated time schedule, but I would encourage you to check back on their website next week. You can also preorder/reserve them now and when they come in, you will be notified and charged. To do this, simply load your cart and pay. There are no promo codes for this option.

    5. You will also notice that when you search for books, it will bring up the same bundle for every class. Keep in mind, you can also choose to purchase the 3 book set online if you prefer a PDF version. This can be purchased at this link through American Nurses Association (ANA):

    Option 2: Elsevier Website

    1. This is where you will go if you only want to purchase the required books and not receive the full bundle. If you purchase this option, you will have to load each book individually into your cart and then use the promo code at the end to get a discount price. This price with the promo code and before taxes is $690.59. You cannot purchase the Essentials of Nursing Practice Package from here, so would need to purchase it either through the TXST Bookstore for the paper version or online via ANA for the PDF version. When added together, the total price for this option is $789.59 before taxes.
    2. The downside to this option is that you are basically paying the same price and not receiving the additional e-supplements, study guides, and resources. It is also an outside vendor of Texas State and not directly from the bookstore which may be more difficult than purchasing them directly from the bookstore. The book bundle from TXST Bookstore is also the better deal, because this is only about  a difference of $5.00.

    Option 3: Purchase through Amazon or other seller (Most risky)

    1. This option can possibly save you money, but is not recommend only because you take the chance of getting the wrong book or editions, etc.
    2. Also, I am still getting several emails from some of you inquiring about having old editions because you purchased them from previous students, etc. I have already stressed the importance of why you need to be careful when purchasing from other students. If there is a new edition of a book for Fall 2020, you will risk the chance of having something similar but not the same exact thing that everyone else has. This could end up hurting you academically. I understand it may also save you money, but new editions are made for a reason and that is to keep you with up to date information as it is published. I would HIGHLY DISCOURAGE you from using the wrong editions. Your faculty also stressed that you need the right editions for the best chance of success when we met via Zoom on Wednesday. For those that were unable to attend that session, I have uploaded it into Canvas under the files listed at Class of 2022 SON Welcome and Meet and Greet, because the file was too large for email. Please feel free to watch it at your convenience.

  • 2020-2022 ATI Fees: ~$1100.00 plus tax

    1st Payment Deadline of ~$275.00 (Fall 2020):  Monday, August 24, 2020 at 5PM


    The St. David’s SON provides you with many resources to be successful in nursing school.  One resource is ATI online resources and testing.  You will access these online resources to clarify complex material you are studying.  Students often benefit from watching videos or reading about complex material in a different textbook. ATI provides you with these resources throughout nursing school.  Each semester you will also take a national ATI assessment which compares your academic progress to nursing students across the nation. Your ATI fees also include a 3-day NCLEX-RN review after you complete your senior year.

    As previously mentioned, You will be required to make this payment every semester. The total cost for all ATI fees is roughly $1100.00 over the course of two years. You will make 4 total payment of ~$275.00. Below is a breakdown of payment deadlines. These payment deadlines will be due on the first day or each semester; therefore will align with financial aid reimbursements if you need to use these funds to submit your payments. You may also choose to pay these earlier than the deadline if desired. You will receive information soon regarding your first payment and how to initiate an account with ATI. Early in the fall semester, a faculty member will demonstrate ATI resources using one of your course syllabi as a guide.
